4. QUALITY CONTROL DOCUMENTATION
Documentation and records are an important and integral part of a rigorous QC Program. Well-kept records
are considered one of the principal forms of objective evidence of quality. Records that demonstrate the
existence and effectiveness of a QC Program are identified and maintained at each project site.

4.1 Onsite Records and Submittals
All onsite records are maintained as prescribed by contract requirements and as deemed necessary to meet
site-specific conditions. Project site personnel are responsible for identifying, collecting, indexing, filing,
storing, maintaining, and final disposition of QC records, including both hard copy (paper) and electronic
(computer stored) documentation. One of the most important forms of documentation in the QC process is
the submittals. Figure 4, on the following page, is a flow diagram for the submittal process.
4.2 Forms and Reports
A number of forms and reports have been developed by Sama Global to support and enhance its QC
program and processes. QC forms and reports suggested for use by on-site QC staff are summarized in Table
1. Examples are provided in Appendix A of this manual.
Variations to the forms in Table 1, and consolidation of material into several stand-alone replacement
reports, are authorized, consistent with the specific contractual requirements for each site. This latitude is
granted to facilitate reporting, effectively economize time and effort applied to report preparation, and
assure information contained in reports is “reader-friendly” to clients.
In addition to the forms in Table 1, which are intended for use at project sites primarily by QCIs, other forms
and reports are also used by PLQCMs or PMs. These forms include the PLQCM’s audit form and after-action
report (used after site visits, but that do not involve formal audits) and the field change request form. The
Performance Assessment Report Form, described in Section 7.0, is also included.
